Job Type: Full-time, Contract (Travel)
Shift: Rotating Day/Night shifts (12-hour shifts)
Schedule: Every other weekend and every other holiday required
Job Summary
Are you a compassionate, reliable LPN looking for your next travel assignment? We are seeking an experienced Travel Long-Term Care (LTC) LPN to join our dedicated healthcare team in Ossian, IA.
In this role, you will provide high-quality, person-centered nursing care, chronic disease management, and routine assessments for residents in a skilled nursing/long-term care facility. We are looking for self-motivated nurses who thrive in a collaborative environment and excel in resident and family communication.
Key Responsibilities
* Monitor resident vital signs, administer prescribed medications, and accurately document treatment responses over time.
* Perform wound care, dressing changes, and routine clinical procedures per individualized resident care plans.
* Continually assess changes in resident conditions and promptly communicate updates to RNs, attending physicians, and family members.
* Assist with activities of daily living (ADLs) and support rehabilitation goals as directed by the care team.
* Maintain accurate, real-time records in the facility’s Electronic Health Record (EHR) system.
* Ensure strict adherence to safety protocols, infection control standards, and facility guidelines.
Qualifications & Requirements
* Education: Graduation from an approved Practical Nursing program.
* Licensure: Active LPN license in the State of Iowa (or a valid Compact/Multi-State LPN license).
* Certification: Active CPR / BLS certification (American Heart Association preferred).
* Experience: Minimum of 1+ year of hands-on experience in long-term care (LTC) or skilled nursing facility (SNF) settings.
* Flexibility: Willingness and ability to work rotating day/night shifts, every other weekend, and every other holiday.
* Skills: Strong communication skills, familiarity with care planning, and proficiency with EHR documentation.
Preferred Qualifications
* Prior travel nursing experience.
* Background or specialized training in wound care management or palliative care principles.
