**Registered Nurse - Home Hemodialysis & Peritoneal Dialysis**
**About the Role**
Henry Ford Health is seeking a compassionate and skilled Registered Nurse to join our Home Hemodialysis and Peritoneal Dialysis program. In this clinic-based role, you will provide specialized nursing care to patients who manage their dialysis treatments at home. You will be instrumental in empowering patients to take control of their treatment while ensuring optimal clinical outcomes and quality of life.
**Key Responsibilities**
As a Home Hemodialysis and Peritoneal Dialysis Nurse, you will:
* Conduct comprehensive patient assessments to evaluate clinical status and identify individualized care needs * Develop and implement customized training plans for patients performing peritoneal dialysis or home hemodialysis * Provide expert patient education on treatment procedures, medication management, and complication recognition and response * Assess and evaluate patients' competency in performing self-administered dialysis treatments and adhering to prescribed medications * Administer medications as ordered by the nephrology care team * Monitor and review laboratory results, home medication regimens, and patient activities to identify clinical changes * Communicate proactively with physicians regarding patient condition changes and clinical concerns * Coordinate patient follow-up care with transplant centers and other healthcare providers * Demonstrate exceptional teaching skills to build patient confidence and independence in home-based treatment management * Prepare patients to recognize and respond appropriately to potential treatment complications

Qualifications
EDUCATION & EXPERIENCE:
* Graduate of an accredited school of nursing. * Possess current RN licensure in the State of Michigan. * The registered nurse responsible for self-care and the home dialysis training must have at least 12 months experience as a registered nurse, plus 3 months experience as a registered nurse in the specific modality of hemodialysis (HD) and/or peritoneal dialysis (PD). * If one registered nurse is responsible for both the HD and PD programs, that nurse must have 12 months experience as a nurse plus at least three months experience in each respective modality. * A charge nurse must have a minimum of 9 months of nursing experience, and an additional 3 months of specialized experience (to total 12 months) providing clinical nursing care to dialysis patients, in either a chronic or acute setting. * Preferred experience in the specialty. * Periodic travel to patient homes required. * Basic computer knowledge and keyboarding skills required.
