The following is a questionnaire and employment/contractor inquiry, and authorization for background inquiry. Please complete it entirely so we can begin your application process. You will only be considered for a possible position when all forms are filled out completely.
Completion of this questionnaire and background check does not guarantee, imply, or contract you as a counselor or direct care provider. If you are chosen as a prospective provider, you will be notified by the administration to start the orientation process.

Responsibilities:
* Provide 1:1 implementation of the individual’s Behavioral Support Plan under the supervision and oversight of a Behavioral Consultant. * Deliver acute, short-term behavioral interventions to address identified challenging behaviors that pose a risk to the individual’s health and safety and/or the safety of others. Maintain focus on immediate stabilization, safety, and risk reduction during behavioral episodes. * Support individuals transitioning from institutional settings or at risk of institutional placement. * Utilize approved crisis intervention strategies, with Safety Care and/or TCI approaches recommended at the time of enrollment. * Provide Behavioral Line Services in the individual’s home and/or in community-based settings, including day programs, by supporting the individual and assisting program staff in maintaining safety and implementing behavioral strategies.
Compensation: $22.62
