Intake Coordinator Qualifications:
* At least 3 years of hospice experience (intake preferred, billing acceptable) in a high-volume referral setting * Receive and promptly process hospice referrals and inquiries, scheduling a hospice representative with the patient and family to assess the need for services * Assign patients to the appropriate team after admission, in coordination with team leaders * Ready to apply hospice experience to the workflows of a centralized system; central office experience is a plus, including handing off and receiving incomplete work items * Strong working knowledge of Teams, SharePoint, and electronic clinical records * Able to work across multiple systems and juggle competing priorities with strong attention to detail * Compassionate, professional phone presence * Reliable transportation, a valid driver's license, and the ability to safely operate a vehicle
Intake Coordinator Schedule: Tuesday to Saturday
* Tuesday to Friday: 10:00 AM - 6:30 PM * Saturday: 11:00 AM - 7:00 PM
