Job Description

Nursing Coordinator

Department: Comprehensive Wellness and Concierge Medicine Schedule: Full-time Ideal Start Date: As soon as possible

About the Department

Join Henry Ford Medical Group, one of Michigan's largest physician organizations, delivering coordinated care across primary and specialty practices throughout the region. As part of Henry Ford Health, you'll benefit from collaborative teamwork, access to leading clinical resources, and opportunities to build lasting relationships with patients and communities.

The Comprehensive Wellness and Concierge Medicine department provides concierge primary care and executive health services for corporate and private-pay patients. Through its concierge program, patients receive personalized primary care and assistance navigating services across the Henry Ford Health system. The team includes internal medicine and family practice providers, nursing staff, and medical support professionals who work together to deliver a coordinated patient experience.

Role Overview

As a Nurse Coordinator, you will play a key role in coordinating personalized care for patients enrolled in the Comprehensive Wellness and Concierge Medicine department's concierge program. Working closely with primary care providers, the medical assistant, specialists, and imaging services, you will help patients navigate appointments, referrals, diagnostic testing, and follow-up care across the health system.

The program currently supports approximately 200 concierge patients, and this new position will add a second nurse coordinator to help share the care coordination workload and support the department's growing patient needs.

This is a patient-facing, coordination-focused ambulatory nursing role that combines relationship-building and care navigation with hands-on clinical support when needed.

* Graduate from an accredited school of nursing. * Current licensure by the Michigan State Board of Nursing as a Registered Nurse. * Current BLS or ACLS certification per Henry Ford Health policy. * Successful completion of the Henry Ford Health Pharmacology test. * Three to five years of progressively responsible clinical nursing practice, consistent with applicable position requirements.

Preferred Qualifications

* Experience in ambulatory care or an outpatient clinical setting. * Experience coordinating patient care across multiple providers, specialties, appointments, and diagnostic services. * Experience in a transplant setting or another complex clinical environment requiring coordination of multiple services and follow-up needs. * Strong customer service, communication, organization, and patient relationship-building skills. * Ability to manage competing priorities, track outstanding care needs, and communicate effectively with patients and multidisciplinary care teams. * Bachelor's Degree in Nursing. * Certification in a relevant specialty.
