What You’ll Do
Provide Patient & Family Support
* Assess patients’ psychosocial needs related to illness, environment, and support systems * Develop and implement individualized care plans focused on emotional, social, and practical needs * Provide counseling, guidance, and support to patients and caregivers * Assist patients and families with navigating resources, financial concerns, and care planning
Connect Patients to Resources
* Coordinate referrals to community resources and support services * Serve as a liaison between patients, families, and community agencies * Build and maintain relationships with local organizations to support patient needs
Collaborate with the Care Team
* Partner with nurses, physicians, and interdisciplinary team members to ensure holistic care * Participate in case conferences and contribute to comprehensive care planning * Support discharge planning to ensure safe and successful transitions
Document & Communicate
* Maintain accurate, timely documentation in the electronic medical record (EMR) * Communicate patient needs and updates clearly with the care team and referring providers * Uphold professional standards and regulatory requirements

What You Bring
* Master’s degree in Social Work (MSW) from an accredited program * Active licensure in the state of practice (as required) * 1+ year of healthcare experience preferred (home health strongly preferred) * Current CPR certification * Valid driver’s license, reliable transportation, and auto insurance
