What You’ll Do:
* Perform in-home patient assessments * Develop and follow individualized care plans * Provide skilled nursing care (wound care, meds, catheter care, chronic disease management) * Educate patients and caregivers on home care and safety * Monitor patient progress and update care plans * Complete timely and accurate documentation

What We’re Looking For:
* Active Texas RN license (required) * Home health or acute care experience preferred * Strong clinical judgment and adaptability * Excellent communication and patient care skills * Reliable transportation & valid driver’s license * Kinnser/WellSky experience (preferred)

Pay & Benefits:
