What You’ll Do
* Complete patient assessments and develop individualized care plans * Coordinate services with physicians, therapists, caregivers, and community resources * Monitor patient progress and adjust care plans as needed * Educate patients and families on medications, treatment plans, and wellness * Keep accurate clinical documentation and maintain regulatory compliance
What You’ll Need
* Active Minnesota RN license (required) * Home health, case management, or transitional care experience (required) * Strong assessment, communication, and care coordination skills * Must be at least 21 years old
