🗓️ Schedule: Monday - Friday 💰 Pay Rate: $25.00 per hour
What You’ll Do:
💼 As a Field Medical Assistant, you’ll play a vital role in driving exceptional patient care and ensuring seamless coordination across our healthcare team—providing oversight, guidance, and exceptional care to a dedicated caseload of patients. You’ll play a vital role in ensuring seamless communication, coordination, and support across providers, patients, and families.
What You’ll Do:
* Conduct engaging patient assessments and develop personalized care plans.
* Collaborate with healthcare providers to deliver smooth and efficient care.
* Manage referrals and follow up with patients to ensure timely care.
* Coordinate communication between patients, providers, and family members—keeping everyone aligned.
* Provide support exclusively in the field!
* Demonstrate professionalism and excellent phone etiquette in every interaction.
* Partner with multiple providers across the Access Healthcare Associates network.
* Monitor patient progress and adjust care plans to fit individual needs.
* Educate and empower patients and families with resources and guidance.
* Apply resource management skills to optimize care delivery.
* Conduct diagnostic evaluations and recommend tailored treatment options.
* Support team goals with additional duties as needed
* Perform other duties as assigned to support patient care and team goals.
Who You Are:
* You have a Medical Assistant Certificate (Required)
