Your Role: Home Health Physical Therapist
You will provide one-on-one care in patients’ homes, helping them regain strength, mobility, and confidence in their daily lives.

Key Responsibilities
* Conduct in-home evaluations, start of care, and follow-up visits * Develop and implement individualized treatment plans * Improve strength, balance, mobility, and functional independence * Educate patients and caregivers on safety and equipment use * Document visits using WellSky Kinnser EMR * Collaborate with physicians, RNs, and the care team

Qualifications
* Active Arizona Physical Therapist license * CPR certification (AHA) * Minimum 1 year of clinical experience (home health preferred) * Strong clinical judgment and time management * Reliable transportation, valid driver’s license, and insurance * Strong communication and a patient-first approach
