Responsibilities
The Collaborating Physician works with an APN (or more than one) who is performing in home and/or telehealth wellness assessments on patients’ designated by HarmonyCares Medical Group. APNs working with the Collaborating Physician will not have prescriptive authority, nor will they be treating patients. As a Collaborating Physician you will:
* Be available via telecommunication or in-person for consultation with APN to address patient assessment questions * Regularly performs annual chart reviews * Consults with APN ensure adherence to policies, procedures, state laws and quality * Be accountable for outstanding customer service to all external and internal customers
Qualifications
* Active/unrestricted Medical Physician License * Board certified/board eligible required * Residency completion required * 1 year experience in Healthcare management * Experience with Risk Adjustment * Must reside in Indiana * Must maintain a valid driver’s license and good driving record
