What You'll Do
* Conduct initial and ongoing in-home client assessments.
* Develop, review, and update individualized care plans.
* Perform caregiver supervisory visits and competency evaluations.
* Collaborate with clients, families, caregivers, and care teams.
* Monitor client conditions and address clinical or safety concerns.
* Maintain accurate documentation and ensure compliance with applicable regulations.
What We're Looking For
Required:
* Active, unrestricted Missouri RN license.
* Current CPR/BLS certification.
* Strong clinical judgment, communication, and organizational skills.
* Ability to work independently and travel to client homes.
* Valid driver's license, reliable transportation, and ability to meet background screening requirements.
Preferred:
* Experience in home health, personal care, or community-based nursing.
* Familiarity with Missouri Medicaid (MO HealthNet) requirements.
* Experience with caregiver supervision and electronic documentation.
Schedule & Compensation
* PRN: Flexible daytime hours, primarily Monday–Friday.
* Hours: Initially limited, with opportunities to increase as client volume grows.
* Coverage: Greater St. Louis, including St. Charles, St. Peters, Maryland Heights, Hazelwood, Webster Groves, Shrewsbury, Richmond Heights, and St. Louis City.
* Pay: Competitive per visit compensation based on experience.
* Mileage: $0.50 per mile for eligible business travel.
PRN hours are not guaranteed, and this position is not eligible for employee benefits.
