Western Geriatrics and Neurology, part of Brigade Health, is hiring compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality mobile primary care to older adults in the comfort of their homes, skilled nursing facilities, assisted living, and senior communities.

What You’ll Do
* Provide mobile primary care visits to seniors * Manage chronic and acute conditions * Perform comprehensive assessments & care planning * Collaborate with caregivers, facilities, and interdisciplinary teams * Deliver compassionate, patient-centered care * Maintain accurate EMR documentation
Qualifications
* Active NP or PA license * 1+ years of primary care, geriatrics, home health, hospice, SNF, or senior care experience preferred * Strong communication and clinical skills * Valid driver’s license & reliable transportation

Pay Range for Posted Region $140,000—$200,000 USD
