Occupational Therapist Responsibilities
As a Home Health Occupational Therapist, you will:
* Complete in-home Occupational Therapy evaluations and reassessments * Develop individualized treatment plans to improve independence with Activities of Daily Living (ADLs) * Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization * Recommend adaptive equipment, home modifications, and fall prevention strategies * Educate patients and caregivers on safety, mobility, and functional independence * Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team * Document patient progress accurately using EMR while maintaining Medicare and agency compliance
Qualifications
* Current Arizona Occupational Therapist (OT) license * Graduate of an ACOTE-accredited Occupational Therapy program * Minimum one year of Occupational Therapy experience (Home Health preferred) * Strong communication, organization, and time management skills * Experience with EMR documentation * Valid driver's license, reliable transportation, and current auto insurance
