Home Health RN Responsibilities
As a Home Health Registered Nurse (RN), you will provide skilled nursing services throughout Maricopa County and surrounding Arizona communities. Responsibilities include:
* Conduct comprehensive nursing assessments in patients’ homes * Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring * Develop, implement, and update individualized home health care plans * Educate patients and caregivers on disease management, medications, safety, and ongoing care needs * Collaborate with physicians, case managers, therapists, and interdisciplinary care teams * Complete accurate and timely clinical documentation in compliance with agency and regulatory standards * Monitor patient progress and communicate changes in condition to the care team * Support improved patient outcomes, independence, and quality of life
Qualifications
To be considered for this RN Home Health position in Arizona, candidates should meet the following requirements:
* Current Arizona Registered Nurse (RN) license in good standing * Graduate of an accredited nursing program (BSN preferred) * Minimum of 1 year of RN experience required * Home Health nursing experience strongly preferred * Experience working with geriatric patient populations preferred * Strong clinical assessment and critical thinking skills * Excellent communication and organizational abilities * Valid driver’s license, reliable transportation, and ability to travel throughout the East and South East Valley area * Ability to work independently while delivering compassionate mobile care
