What You Will Do
As an RN Case Manager, you will provide skilled nursing care in the home while guiding each patient’s plan of care from start to finish.
* Assess, plan, implement, and evaluate individualized patient care * Develop care plans in collaboration with physicians and the interdisciplinary team * Complete admissions and ongoing nursing assessments * Educate and support patients and families through changing needs * Document care accurately and in a timely manner

Salary Range: $72,000 to $90,000 annually, based on experience
What You Bring
* Active Missouri RN license in good standing * Home health RN experience preferred * Strong clinical judgment and documentation skills * Ability to manage a flexible schedule including admissions * Comfort with technology and electronic medical records
