Home Health RN Responsibilities
* Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments * Develop and implement individualized home health care plans * Monitor patient progress and adjust care plans as needed * Educate patients and families on disease management, rehab, and senior care * Coordinate care with physicians, therapists, and the Director of Nursing * Document patient visits in compliance with home health regulations

Qualifications – Home Health RN Jobs
* Active California Registered Nurse (RN) license (required) * Minimum 1 year RN experience (home health experience preferred) * Experience in rehab, geriatric care, or community health is a plus * Strong clinical, communication, and organizational skills * Valid driver’s license and reliable transportation * Ability to travel throughout Orange County
