Affirmed Home Care is seeking a compassionate, dependable, and skilled Licensed Practical Nurse (LPN) to join our team and provide high-quality, one-on-one care to a client in Newark, New Jersey.

What You’ll Do
* Provide compassionate nursing care according to the client’s individualized care plan
* Administer medications and prescribed treatments
* Monitor the client’s condition and promptly communicate any changes
* Provide wound, catheter, and other skilled nursing care within your scope of practice
* Maintain accurate and timely clinical documentation
* Promote the client’s comfort, safety, dignity, and independence
* Communicate professionally with the client, family members, and care team
What We’re Looking For
* Active New Jersey LPN License
* 1+ year of nursing experience preferred
* Current CPR/BLS certification
* Strong clinical, communication, and organizational skills
* Reliable, compassionate, and professional
* Comfortable working independently in a home care setting
* Valid driver’s license and reliable transportation preferred
* EHR/documentation experience preferred
