What We Offer:

* $28.00 – $32.00 per hour, depending on experience

Job Responsibilities:
* Conduct in-home client assessments and develop individualized plans of care
* Serve as a clinical liaison between caregivers, physicians, families, and the office team
* Train, guide, and support caregivers on care plans and safe practices
* Monitor client progress and revise care plans as needed
* Ensure compliance with state home care regulations and agency policies

Qualifications:
* Current RN license in the state of Georgia (Required)
* BLS Certification (Required)
* 2 years of RN experience in home health, geriatrics, or related field (Preferred)
* Excellent clinical and communication skills
* Reliable transportation, valid driver’s license, and auto insurance
* Clean background and driving record
