Your Role as a Licensed Practical Nurse (LPN)
As an LPN with Home Sweet Home, you will deliver hands-on skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN).
Primary Responsibilities:
* Provide skilled, patient-centered nursing care based on individualized care plans
* Accurately document all visits, assessments, and treatments using electronic health records (EHR)
* Communicate effectively with patients, families, and the home health care team
* Ensure compliance with Arizona home health regulations and company policies
* Help patients meet their health goals through compassionate and professional nursing

Qualifications for Home Health LVN Jobs
* Current Arizona Licensed Practical Nurse (LPN) license in good standing. * Graduate of an accredited vocational nursing program * Minimum 1 year of nursing experience (home health experience preferred) * Physical ability to assist and move patients as needed * Strong communication and documentation skills * Proficiency with electronic charting and care coordination software * Valid driver’s license, reliable transportation, and ability to travel to patient homes in Central Phoenix and surrounding Maricopa areas.
