What You’ll Do:
* Evaluate patients and develop individualized therapy plans * Provide in-home physical therapy treatments * Improve mobility, strength, balance, and functional independence * Educate patients and caregivers on exercises and safety techniques * Monitor progress and adjust treatment plans as needed * Complete accurate and timely documentation

What We’re Looking For:
* Active Texas Physical Therapist (PT) license (required) * Home health experience preferred * Strong clinical judgment and patient care skills * Ability to work independently in a home setting * Excellent communication and documentation skills * Reliable transportation & valid driver’s license * Kinnser/WellSky EMR experience (preferred)
