What You Will Do

* Collaborate with the healthcare team to identify and assess patient needs. * Develop and implement care plans that promote patient safety and quality of care. * Coordinate services and resources to support patients' health outcomes. * Provide effective discharge planning to ensure a safe transition to the next level of care. * Advocate for patients and families, addressing their needs and concerns. * Evaluate care plans and outcomes to optimize health along the care continuum.

Qualifications

What You Will Need

Required Qualifications

* Bachelor's degree in nursing or a Master's degree in Social Work. * Minimum 3 years of clinical experience. * Excellent verbal communication and written documentation skills. * Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.

Preferred Qualifications

* Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC). * Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
