Henry Ford Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide high-quality home health care services throughout designated Detroit communities. Downtown Detroit Area: 48201, 48202, 48204, 48206, 48208, 48216, 48221, 48226, 48238
Schedule:
* Full-Time * Monday-Friday * 8:00 AM - 4:30 PM * 1 Saturday a month.
Responsibilities
* Deliver skilled nursing care in patients' homes under the direction of the Registered Nurse and care team. * Monitor patient conditions and report changes to the appropriate healthcare providers. * Administer medications and treatments as ordered. * Educate patients and family members on disease management and care plans. * Complete accurate and timely documentation. * Travel throughout the assigned service area to provide patient care.

What You Will Need
Required Qualifications
* Successful completion of a Licensed Practical Nursing (LPN) program accredited by the National League of Nursing. * Minimum of one (1) year of hands-on clinical LPN experience. * Current Michigan Licensed Practical Nurse (LPN) license in good standing. * NAPNES certification or successful completion of a pharmacology examination. * Reliable vehicle for daily travel to patient homes. * Valid Michigan driver's license and satisfactory driving record.
Preferred Qualifications
* Previous Home Health Care experience.
