What You'll Do * Conduct occupational therapy evaluations and provide treatment within patients’ homes * Create personalized care plans designed to promote independence * Instruct patients and caregivers on safety techniques, adaptive methods, and equipment use * Complete documentation in a timely and accurate manner * Share updates on patient progress with the care team across disciplines * Ensure adherence to all clinical guidelines and documentation requirements
What We're Looking For * Active Occupational Therapist (OT) license * Prior home health experience is a plus, but not mandatory * Excellent verbal and written communication abilities * Capable of working autonomously while effectively managing your schedule * Dependable, professional, and committed to delivering patient-centered care
